AI Outbound Calling

Cost model sourced to BLS wage data and published vendor rates

Updated

Patient communication is relentless, repetitive and regulated. Reminder calls, recall outreach and post-discharge follow-up are known to improve outcomes and revenue, and they are the first thing dropped when staff are stretched.

What It Actually Costs To Run This Manually

The cost comparison, with sources

A human agent is paid for every minute they are on shift. An AI agent is billed only for minutes it is actually on a connected call. That difference — not the headline hourly rate — is where the saving comes from, and it means the honest comparison depends entirely on your agent utilisation.

Human agent

$21.53/hour

$0.36 per minute of paid time — whether or not anyone answers.

US median wage for Customer Service Representatives, $44,770/year (BLS, 2025). Wage only — excludes payroll tax, benefits, software, supervision and facilities.

AI agent

$0.05–$0.50/min

Per minute of connected call. Cheapest published rate: Vapi at $0.05. Most expensive: TheLeadRouter at $0.50.

Published voice rates from the 6 platforms in our review that disclose one. Several bill model, telephony or transcription costs on top — see each review for the all-in figure. The other 8platforms publish no per-minute voice rate at all.

Effective human cost per minute of live conversation

If agents are on live calls…Cost per talk-minutevs AI at $0.05/min
20% of paid time$1.7936× the AI rate
35% of paid time$1.0321× the AI rate
50% of paid time$0.7214× the AI rate

How to read this: utilisation is the share of paid time an agent spends in live conversation, after dialling, no-answers, voicemails, wrap-up and breaks. Use your own figure — we deliberately do not publish an industry average, because it varies enormously by list quality and dialler type. The AI column is not free either: it excludes build time, telephony and the human handling every transfer the agent passes on.

Where The Cost Comes Out

Cost reduction in this industry is not a single number. It comes from three or four specific places, and knowing which apply to you determines whether a deployment pays back.

Reducing no-show rate

No-shows waste clinician time, which is the most expensive resource in the practice. Consistent confirmation measurably reduces them.

Recall outreach that currently does not happen

Preventive-care recall is both clinically indicated and revenue-generating, and it is almost always under-run.

Staff time returned to in-person patient care

Reminder calling is the clearest example of work that does not require a clinical person.

Where The Predictability Comes From

The cost argument gets the attention, but operators who have run this for a while tend to say the bigger win is variance removal. US employers post roughly 289,500 customer-service openings a year largely to replace turnover, and the occupation is projected to shrink 5% through 2035 (BLS, 2025) — staffing a phone line is getting harder, not easier.

Every patient gets the same reminder cadence

Reminder coverage stops varying with staffing levels.

Recall lists are worked completely

A recall cohort gets contacted in full rather than until someone ran out of time.

What To Automate, What To Keep Human

The deployments that fail are almost always the ones that automated a conversation requiring judgement. The line below is where we would draw it in this industry.

Hand to an agent

  • Appointment reminders and confirmations
  • Preventive-care and recall outreach
  • Prescription refill reminders
  • Post-discharge check-in calls on a fixed script

Keep with a person

  • Any symptom description or clinical triage
  • Test results of any kind
  • Medication changes
  • Any call where the patient sounds unwell or distressed

Best Practices

A signed BAA is the entry requirement, not a nice-to-have

Patient data is PHI end to end. Note that pricing differs sharply here: some vendors include a BAA, others charge separately for HIPAA — one lists it at $2,000/month. Budget for the compliant tier, not the headline rate.

Script post-discharge calls tightly and escalate on anything clinical

These calls are valuable precisely because they catch deterioration. The agent's job is to detect and escalate, never to assess.

Confirm the BAA covers recordings and transcripts

Call audio and transcripts are PHI. A BAA scoped only to API traffic leaves your largest data store uncovered.

Do not automate results delivery

Even normal results carry clinical context. This is a human conversation.

Compliance Exposure

PHI throughout. A signed HIPAA BAA is mandatory, and 'HIPAA-ready' is not the same thing. Note that some vendors charge separately for HIPAA — Vapi lists it at $2,000/mo. Bland offers a signed BAA, and Synthflow adds ISO 27001 with EU hosting.

General information, not legal advice. Consult counsel before running outbound campaigns in a regulated vertical.

A Rollout Sequence That Works

Order matters more than platform choice. Every step below is chosen so that the previous one has already proven the agent can be trusted with the next.

  1. Execute a BAA and confirm transcript and recording coverage

  2. Deploy reminders and confirmations only

  3. Add preventive-care recall once reminder accuracy is proven

  4. Add post-discharge check-ins with a hard clinical escalation path
